By participating and signing up for ATMN Christmas Wish you understand and agree to the following:

1. You will only register your child/children for one Christmas present program. Should you choose to participate in more than one you may become ineligible for participation in the ATMN Christmas Wish program moving forward.

2. This form DOES NOT GUARANTEE your acceptance to Christmas Wish. Once your paperwork has been received you will receive a call from ATMN staff to review and confirm.

3. Upon acceptance into the program, you agree to write your sponsor a thank you card before the end of January, 2024. If you need assistance, please contact ATMN staff.

4. Christmas Wish program is for active and in need participants of ATMN youth programs.

5. You must re-apply and resubmit this form each year to participate in our Christmas Wish program.

All Things Made New may share my information applicable to the Christmas Wish Program with my sponsor, members, donors and other supporters in fundraising/marketing materials.
